Disability Documentation
Disability documentation can be submitted to KOKUA via UH FileDrop or fax.

Documentation sent to KOKUA is reviewed by KOKUA professional staff and assists in determining appropriate accommodations. Documentation is kept in confidential files at the KOKUA Program office. Documentation will vary depending on the classification of disability. If documentation is determined to be insufficient, KOKUA may request additional documentation. Disability documentation written in a language other than English must be translated and notarized at the expense of the student.

We ask that students retain their own copy of their disability documentation for future use as KOKUA will not indefinitely house this material and is unable to provide copies of documentation.
Furnishing false or fraudulent information is considered an act of dishonesty as stated in the University’s Student Conduct Code (EP 7.208), and conflicts with the community standards that UH values and expects of students. Engaging in, or attempting to engage in any of these behaviors subjects a student to the disciplinary process and sanctions on each campus.
