Case 3

A 4 day old infant of a constantly moving homeless family presents to your clinic for the first time. The history and physical exam is otherwise normal.  On review of systems, you learn that the baby is sleeping in a sleeping bag with the mother.   

Questions to think about/discuss:  

1. What are the problems in this case?
2. What medical home principles need to be addressed in this case?
3. What outside agencies or individuals would you call?
4. What actions would you take to establish a medical home for this child?
